Apply now for the Oregon Health Plan

Oregonians can enroll year-round

Individuals and families can apply for OHP at any time. If qualified, you will be enrolled automatically and your coverage could start immediately. There is no special enrollment period required.

There are two ways to apply

Please read the important information below before you get started.

Application Checklist

What you need to get started on your application:

  • Birthdate(s)
  • Social Security number(s) (for those who have one)
  • Income and deduction information (for example, from pay stubs or W-2 forms)
  • Alien Resident number(s) (for those who have one)
  • Information about health insurance available to you through an employer

Important information

You will need to complete a paper application if there is anyone in your household who was receiving foster care in Oregon when they turned 18 and you are applying for coverage for this person. See the instructions below on how to download an application.

Submit your application online

Before you fill out and submit your online application, first read the important information below about the web browser you should use. If you use a web browser that does not support the online application, it can result in an incomplete application.

Important information

Required web browsers to submit the application online

  • Internet Explorer (version 9+) on Windows (PC) - Works best with Adobe Reader 9 or higher
  • Safari (version 6 and lower) on OS X (Macintosh) - Works best with Adobe Reader 10 or higher.

Adobe Acrobat Reader

Adobe Reader is needed to complete the online and printable application. If you don’t have Adobe Reader installed, follow the link below to download and install it on your computer.


Important

The online PDF application does not save information you entered. Do not use the “Back” button or navigate away from the page while you are filling out the application. If you do, you will lose information you already entered.

 


Print your application and mail or fax it

Adobe Acrobat Reader is needed to complete the printable application. You can print it and fill it out by hand. Or you can fill it out on your computer and then print it. Please note that the only way to save it to your computer is by using the “print” option and saving the pdf.

 


Before you send it to us: Make sure all necessary pages are filled out and SIGN it before sending it to the address on the application.

When you are done you can mail or fax it to us.

If you don’t qualify for OHP, you can still get insurance

If you apply for OHP using the state application, the Oregon Health Authority will notify you by mail that you did not qualify for OHP. You can then apply at HealthCare.gov, where you can find out if you qualify for private health insurance and financial help to lower the cost of coverage.
